Acute Care Certificates are required to be completed by a doctor for any acute hospital stays over 35 continuous days, and then every 30 days thereafter and submitted to member funds by the hospital.

The form is completed to verify the type of patient (i.e acute care or other). Most patients who require stays more than 35 days are nursing home type patients (NHTP), and for which services are paid at a different rate compared to  to an acute care patient.
